Request for Student Records for Deferred Action Process
If you are applying for consideration or renewal of deferred action for childhood arrivals (DACA), you need records dating back to June 15, 2007. Complete the form available by clicking the link above and return it to the Instruction Department, 200 N. Monroe St., Eugene OR 97402. You can send the completed form via email to enroll@4j.lane.edu or via fax to 541-790-7711. You will be contacted when your records are ready to pick up, or if more information is needed.
