Request for Student Records for Deferred Action Process

If you are applying for consideration or renewal of deferred action for childhood arrivals (DACA), you need records dating back to June 15, 2007. Complete this form and return it to the 4J District Registrar, 200 N. Monroe St., Eugene OR 97402. Phone: 541-790-7712 or Fax: 541-790-7711. You will be contacted when your records are ready to pick up, or if more information is needed.

Deferred action has changed since this process was implemented in 2012. For more information about deferred action, see U.S. Citizenship & Immigration Services.
