4J Staff Flu Shot Clinics 2020

School and district staff: Protect yourself against the flu!

Flu shot clinics for staff on October 8th, October 14th and October 20th

Consent Form | Vaccine Information | Información Sobre Vacunas

Getting a flu shot is the best protection we have from the flu and its complications, although it cannot prevent all cases of the flu. A flu vaccine also helps prevent spreading flu from person to person.

4J will hold three flu shot clinics this fall for you and your eligible dependents (age 10 and over) to get this year’s vaccination.

The staff administering the vaccine will need easy access to your upper arm—specifically the deltoid muscle below the shoulder. Please dress accordingly.

4J flu shot walk up clinics will be located outside at the Ed Center at 200 N. Monroe St.

• Thursday, October 8, 1:30–5 p.m.
• Wednesday, October 14, 1:305 p.m.
• Tuesday, October 20, 3–5 p.m. 2:30–5:30 p.m. (new time)

Flu Shot Walk Up Clinic MapDue to the coronavirus pandemic, the clinics will be held outside.
– Please wear a face covering at all times and, while in line, stay on the taped markers on the ground to maintain 6′ distance from anyone not in your immediate household.
– Please have the consent form filled out prior to your arrival as pens and clipboards will not be provided.

In this case, “dependents” refers to spouses and domestic partners, as well as insurance eligible children age 10 or over. All children 10–16 years of age must have a parent or guardian with them to be vaccinated. The U.S. Centers for Disease Control and Prevention (CDC) recommends that all people 6 months of age and older should get a flu vaccine, but there are different protocols for young children. The particular vaccine we will have available is not approved for children under 10, so if you have children you would like to get vaccinated who are not eligible for the 4J clinics, you will need to contact your pediatrician.

Flu shot eligibility extends to:
• all active employees (even those who are not insurance eligible due to less than half-time status)
• all insurance eligible dependents of active employees, including those who have waived coverage
• all retirees and their dependents who are currently covered on the 4J insurance
• student teachers and substitutes (licensed and classified)

Flu shot eligibility does not extend to:
• retirees who are no longer on the 4J insurance
• retirees’ dependents who are not on the 4J coverage (even when the retiree is still covered)
