Eugene School District 4J partners with OEBB to administer the COBRA insurance program.  If you are in a situation where you no longer have benefits with 4J and you are eligible for COBRA, you will receive detailed information and instructions at your home address.  A few important things to remembers:

• Please ensure that the district has your current mailing address on file.
  • If you need to update your address please fill out the change form located here and return it to the HR office.
• It is very important to read all of the material sent to you and return any paperwork and/or payments to the appropriate places on time.
