Important Information Regarding Leaves During the COVID-19 Pandemic

During the COVID-19 pandemic, staff who need to complete a leave request because of COVID-19 can submit a shortened version of the district Leave Request by filling out this Google Form (click here to complete a leave request).  Please note you will need to be logged into your 4J account in order to access this form.  A guide regarding how to sign out of a personal google account and log into your 4J Google account can be found here if you are having difficulties completing the form. Leaves not related to COVID-19 must be submitted using the standard leave request below.

In March the Federal Government passed the Families First Coronavirus Response Act (FFCRA) which grants additional paid leave under certain circumstances related to COVID-19.  Eligible employees may receive additional paid leave under the FFCRA if they are not able to report to work for reasons such as experiencing symptoms of COVID-19 and seeking a diagnosis or need to care for their child whose school or daycare was closed due to COVID-19.  More information about the FFCRA can be found by clicking here.  The Human Resources team is able to help answer questions about the FFCRA too.  Contact us anytime at HR_Leaves@4j.lane.edu.

Leave of Absence Request Forms

Medical Leave of Absence Packet = Leave to care for one’s own serious health condition.

Family Medical Leave of Absence Packet = Leave to care for an immediate family member who has a serious health condition.

Non-Medical Leave of Absence Packet = All leave types not pertaining to medical necessity. This includes bereavement leave, personal leave, part-time leave, professional leave, parental leave (to care for your newborn child, newly adopted child, or to provide foster care),  and all other leaves allowed under the District/EEA Bargaining Agreement, the District/OSEA Bargaining Agreement, or the District/4JA Statement of Understanding.

Unpaid Days

Leave Request for Unpaid Days = Unpaid leave equal to or less than five (5) workdays in length.

  • Note: if you are requesting a leave longer than five (5) workdays, you will need to complete one of the regular leave forms listed in the previous section above.
