Wellness Clinic


The Wellness Clinic will be open regular business hours during the school closure due to the Coronavirus, as well as during 4J’s regular spring break, March 23-27.  The Clinic would like to pass along the following information:

The Wellness clinic has instituted protocols to keep patients and staff safe during the Covid19 pandemic.

  • The lobby is closed and walk-ins are not accepted. Please call ahead for scheduling. Knock on the door or call when you arrive
  • Temperature screenings upon entry into the building and face masks/ coverings are required.
  • Patients are asked not to bring additional visitors. We ask only one parent per minor child be in attendance.
  • Televisits (video charts) and telephone calls are optimized to decrease routine need for clinic visits.
  • In person appointments are spaced out to decrease the cross traffic of patients.
  • Ill patients are often provided services through video, curb side evaluation and at the end of the day.
  • Lab follow ups are often electronic and available for view by the patient
  • Staff has increased routine personal protective equipment (PPE)
  • New patient and annual physical forms are available electronically prior to appointment time.
  • Mental Health services are available through video or off site (counseling is currently not being offered on site).
  • Routine and preventative care is being offered.
  • Flu shots are going to be available mid to late September
  • Routine Covid19 testing is being ordered through our community partners McKenzie Willamette Drive- Thru (a provider’s order is required).
  • Potential aerosolizing procedures of nebulizer medication delivery and spirometry are currently not being performed in clinic.

Wellness Clinic General Information:

The Wellness Clinic is a medical clinic run through a joint effort of the District and its employees (via the Joint Benefit Committees) to provide insurance-eligible 4J employees, retirees and their families with pre-paid medical care for routine needs. *NEW* 4J employees and retirees who enroll in 4J/OEBB Kaiser Health as their primary health insurance coverage will be eligible to use the clinic as of 10/01/2020. The clinic has two nurse practitioners, a mental health provider and support staff who work together to provide high-quality care.  There is no out of pocket cost for visits to the clinic but your insurance will be billed.  Visits are scheduled by appointment only, just as you would your regular physician.

Clinic services are provided by Cascade Health.  Cascade Health  is honored and excited to have the opportunity to work with you and your family through the 4J Wellness Clinic. Getting set up for your first 4J Wellness Clinic visit is easy. For new patients, please follow the three easy steps below:

  1. Call (541) 686-1427 to schedule your appointment.
  2. Have your health insurance information available when scheduling your appointment, even if you are not insured through the Eugene School District 4J’s insurance.
  3. You will need to fill out new patient paperwork for your first visit. You can have the paperwork mailed to you, pick up the paperwork during clinic hours prior to your appointment, or arrive 15 minutes early to fill out new patient paperwork on the day of your appointment.

For existing patients:

  1. Call (541) 686-1427 to schedule your appointment.
  2. If your health insurance information has changed, please have the new information available when scheduling your appointment.
  3. If there are any changes that need to be made to your records, such as an address or phone number change, please arrive 15 minutes early to update your records, or inform the clinic of the changes when scheduling your appointment.

Clinic Services

The clinic provides a full range of primary health care, diagnostic tests, minor surgery and preventative care. Annual physicals are available, as well as school, sports and camp physicals for children. See below for a listing of services.

• Child well exams and sports physicals; age 4 years and older

• Lesion /mole and wart removal; punch biopsy of skin if indicated
• Abscess incision and drainage
• Suturing of lacerations (dependent on the body site)

• Nail removal
• Foreign object removal (i.e. splinters/glass)
• Muscle strains/sprains (non-emergent body injury)
