Wellness Clinic


The Wellness Clinic will be open regular business hours during the school closure due to the Coronavirus, as well as during 4J’s regular spring break, March 23-27.  The Clinic would like to pass along the following information:

  • Well patients will be scheduled for morning appointments
  • Sick patients will be scheduled for afternoon appointments
  • Patients who are ill will be phone screened prior to scheduling an appointment to determine if they need to come to the Clinic
  • If you have scheduled a well-check appointment and become ill, please call the Clinic to arrange for an afternoon appointment.
  • Call your pharmacy if you need medications refilled, even if there are no refills remaining.
  • The Clinic is working on telemedicine availability
  • It is not too late to get a flu shot, and the Clinic has flu vaccines available
  • Please wait to schedule routine care visits, and if you already have a physical scheduled, consider postponing
  • Behavioral health appointments will be scheduled at the Cascade Health office, located in Eugene at 2650 Suzanne Way, Suite 120

Wellness Clinic General Information:

The Wellness Clinic is a medical clinic run through a joint effort of the District and its employees (via the Joint Benefit Committees) to provide insurance-eligible 4J employees, retirees and their families with pre-paid medical care for routine needs. 4J employees and retirees who enroll in 4J/OEBB Kaiser Health as their primary health insurance coverage are not eligible to use the clinic. The clinic has two nurse practitioners, a mental health provider and support staff who work together to provide high-quality care.  There is no cost for visits to the clinic and visits are scheduled by appointment only, just as you would your regular physician.

Clinic services are provided by Cascade Health.  Cascade Health  is honored and excited to have the opportunity to work with you and your family through the 4J Wellness Clinic. Getting set up for your first 4J Wellness Clinic visit is easy. For new patients, please follow the three easy steps below:

  1. Call (541) 686-1427 to schedule your appointment.
  2. Have your health insurance information available when scheduling your appointment, even if you are not insured through the Eugene School District 4J’s insurance.
  3. You will need to fill out new patient paperwork for your first visit. You can have the paperwork mailed to you, pick up the paperwork during clinic hours prior to your appointment, or arrive 15 minutes early to fill out new patient paperwork on the day of your appointment.

For existing patients:

  1. Call (541) 686-1427 to schedule your appointment.
  2. If your health insurance information has changed, please have the new information available when scheduling your appointment.
  3. If there are any changes that need to be made to your records, such as an address or phone number change, please arrive 15 minutes early to update your records, or inform the clinic of the changes when scheduling your appointment.

Clinic Services

The clinic provides a full range of primary health care, diagnostic tests, minor surgery and preventative care. Annual physicals are available, as well as school, sports and camp physicals for children. See below for a listing of services.

• Breast exams/ cancer screening
• Child well exams and sports physicals; age 4 years and older
• Complete male annual exams/prostate exam
• Complete female annual exams/gynecological exams; Pelvic/ Pap smear/HPV testing

• Animal bites/scratches
• Lesion /mole and wart removal; punch biopsy of skin if indicated
• Abscess incision and drainage
• Suturing of lacerations (dependent on the body site)
• Suture removal
• Nail removal
• Foreign object removal (i.e. splinters/glass)
• Muscle strains/sprains (non-emergent body injury)
