Enrollment for Students in Shared Housing

2020–21 Shared Housing Request

Shared Housing

When a family is living in an informal housing arrangement, such as with a relative or friend, or when the parent/guardian of the student cannot provide the Proof of Residence/Address documents legally required to enroll their student, then a Shared Housing form is another way to verify a student’s Proof of Residence/Address for enrollment in a Eugene School District 4J school.

Shared Housing Form Process

The Shared Housing form is available through the School Choice and Student Placement Office. Completed original forms must be mailed or delivered to: Student Placement Coordinator, Eugene School District 4J, 200 North Monroe Street, Eugene, OR 97402.

  1. Obtain a Shared Housing form
    • Submit an online request for a Shared Housing form, or
    • Email schoolchoice@4j.lane.edu to request a Shared Housing form.
    Upon receipt of the request, the School Choice and Student Placement Office will email you a list of the required documentation and next steps for 2020–21 enrollment in Eugene School District 4J schools.
  2. Get signatures notarized on Shared Housing Form with an Oregon Notary Public. The parent/guardian and the homeowner/leaseholder signatures on the Shared Housing form must be notarized. Both parties must bring a current driver’s license or other valid picture identification to have their signature notarized. Free notary services are available through the Eugene School District 4J School Choice Office. Please email schoolchoice@4j.lane.edu for more information.
  3. Attach a copy of ONE of the following documents recently issued or currently valid, which includes the name and residential address of the homeowner/leaseholder:
    • Mortgage statement
    • Lease agreement
    • Rent receipt
    • U
    tility bill, or
    • R
    ecently mailed document from government entity, e.g. State of Oregon or United States agency.
  4. Mail or drop off a completed, notarized Shared Housing form with one of the above documents to:
    School Choice and Student Placement Office, 4J Education Center
    200 North Monroe St.
    Eugene, OR 97402.
    The Ed Center remains closed to the public due to the coronavirus pandemic. A mail slot to drop off completed Shared Housing forms is available at the front entrance.

Loss of Housing, Temporary Living Arrangement or Homelessness

If the shared housing is a temporary living arrangement due to loss of housing or economic hardship, please contact the 4J McKinney-Vento office, 541-790-4932, for more information regarding eligibility for additional educational services under the McKinney-Vento Education Act.
