2020 Student Enrollment

Enroll your student now for the 2020–21 school year

Welcome to Eugene School District 4J. We are pleased to have you joining us!

With school buildings closed due to the coronavirus pandemic, school enrollment is a little different for the 2020–21 school year.

To enroll a new student you may submit enrollment forms and documents online or on paper.

Digital Registration 

(1) Download: Download the registration packet (fillable PDF) here: English / español , Oregon Certificate of Immunization Status , 4J_Annual_Permissions_2020-21 , 4J_G_Suite_Notice_Permission_Form , and 2020-21-Annual-Notice-of-Legal-Rights-and-opt-out-forms .

(2) Complete: First, you must download the PDF (saved on your computer), then fill it out and save it. Do not fill in the PDF in a web browser, as the information you enter will not be saved. Please complete all of the forms in the packet and save the PDF.

(3) Document: Make a digital copy (scan or photos of the entire document) of your proof of age and proof of address and parent or guardian identification. (You may be asked to verify these documents in person at a later time.)

(4) Return: Submit your completed registration forms, your proof of age and address, Oregon Certificate of Immunization and copy of parent or guardian identification by email. Please make sure to include the name of your student, grade and school enrolling at in the body of your email.

  • Grades K–8:
    For elementary enrollments, please email your forms and documents to your school’s secretary.
    For middle schools, please email your forms and documents to your school’s registrar.
    (Elementary and middle school operations close for summer break on June 24 and reopen in August before school begins.)
  • Grades 9–12:
    Email your completed registration packet to your school’s registrar.
    (High school operations do not close for summer. Please note that ECCO is a referral-only program. If you are interested in ECCO, please contact your neighborhood high school registrar.)

Paper Registration 

(1) Get the packet:

  • Pick up: Paper registration packets are available at student meal sites while grab-and-go meals are being distributed, Monday–Thursday, 11 a.m.–12 p.m.
  • Mail: To request a registration packet to be mailed to you, please contact your school or the district registrar at enroll@4j.lane.edu

(2) Complete: Please complete all of the forms in the packet.

(3) Document: Make a copy of your proof of age and proof of address and parent or guardian identification. (You may be asked to verify these documents in person at a later time.)

(4) Return: Submit your completed registration forms and your proof of age, address and parent or guardian identification, and immunization records.

  • Drop off: You can return your completed registration packet to the District Registrar, 4J Education Center, 200 N. Monroe St. (mail slot is available at the front entrance)
  • Scan and send: You also may scan your completed registration packet and documentation of age, address and parent or guardian identification, and return it by email as listed above in the digital registration section.
