4J Vaccine Clinic for Students Age 12+ on May 17

COVID-19 vaccine is expected to be authorized for youth age 12–15 soon • First 4J student vaccine clinic for ages 12+ on May 17

UPDATE: All of the May 17 vaccine appointments have been filled. Do not contact the district or healthcare provider about vaccine appointment availability. Additional vaccine clinics will be scheduled.

Eugene School District 4J has scheduled a COVID-19 vaccine clinic for 4J students age 12 and up on Monday, May 17. FDA authorization of the Pfizer vaccine for youth age 12+ is expected soon.

Vaccination is a voluntary and important opportunity to support the health of our families, schools and Eugene community. 4J is joining with community providers to make COVID-19 vaccines available at no cost for students. This is the second vaccine clinic scheduled solely for 4J students, and the first for ages 12–15. Another clinic for the second vaccine dose will be on June 7.

Who: 4J students age 12 and up
 South Eugene High School, 400 E. 19th Ave.
When: Monday, May 17, 1–6 p.m. (pending FDA authorization for age 12+)
Make an Appointment: Make an appointment for each student, under the student’s name

Parent consent: Students under age 15 must be accompanied by their parent or guardian or bring a signed consent form.

Insurance information: If you have insurance, you will need to bring your insurance card or information. If you don’t have insurance, there will be no charge for the vaccine.

Pending approval: At this time COVID-19 vaccines are available for all Oregonians age 16 and older. Federal authorization of the Pfizer vaccine for use in youth age 12–15 is under consideration and anticipated to be granted soon. If the FDA has not given authorization for the vaccine to be administered to youth age 12 and up before May 17, the district will reschedule this vaccine clinic.
