4J Hosts Ongoing COVID Vaccine Clinics

COVID-19 vaccine authorized for all Oregonians ages 12+ • 4J hosting clinics with appointments and walk-in availability

Vaccination is a voluntary and vital opportunity to support the health of our families, schools and community. All Oregonians age 12 and up are now eligible to receive the COVID-19 vaccine. The FDA has authorized use of the Pfizer vaccine for people age 12 and older. Other vaccines are available for adults age 18 and older.

Eugene School District 4J is partnering with community providers to make COVID-19 vaccines available at no cost for staff, students, families and community members.

Lane County Public Health Vaccine Clinics at 4J Schools

4J is partnering with Lane County Public Health to provide ongoing vaccine clinics at 4J school sites, open to students, families, staff, and other community members. Advance appointments are available, or community members can drop in to get their shot.

When: Wednesdays starting May 19, 4–6 p.m.
Where: Churchill High School Cafeteria, 1850 Bailey Hill Rd.

When: Fridays starting May 21, 4–6 p.m.
Where: North Eugene High School Glass Lounge, 200 Silver Lane

Appointment or walk-in: While appointments are encouraged and will reduce wait times, walk-ins also are welcome at LCPH vaccine clinics. Schedule Appointment

Parent consent: Youth under age 15 must be accompanied by their parent or guardian or bring a signed consent form.

Eugene School District 4J Vaccine Clinics

4J also is working with a healthcare partner to provide vaccine clinics for 4J students, families and community members age 12 and older. Some appointments are still available on May 24.

When: Monday, May 24, 1–5 p.m.
South Eugene High School Cafeteria, 400 E. 19th Ave.

Advance appointment: Please make an appointment in the name of the person getting the vaccine, and be sure to log back in and cancel if you no longer need the appointment. Schedule Appointment

Parent consent: Please bring a completed consent form. Extra copies will be available on-site if needed. Youth under age 15 must be accompanied by their parent or guardian. Consent Form

Insurance information: If you have insurance, please bring your insurance card or information. If you don’t have insurance, there will be no charge for the vaccine.
