  • 4J Wellness Clinic Smoking Cessation Program:

    The 4J Wellness Clinic offers a smoking cessation program that is available to all benefits-eligible employees and their benefits-eligible dependents.  People who successfully quit smoking may be reimbursed for the cost of tobacco cessation products (nicotine gum or patches, certain prescriptions) that you use in this program.  To be eligible for financial reimbursement under the 4J Wellness Clinic program:
    1. Schedule an appointment with the Clinic to discuss your desire to quit smoking.
    2. Together with the Wellness Clinic Nurse Practitioner, you decide the best method for you.
    3. Set a quit date, and the Clinic Nurse Practitioner will determine when to start any medication, and when to return for follow-up visits.
    4. Save all receipts for prescriptions, nicotine patches or gum.  After 3 months without smoking, a Clinic Nurse Practitioner will give you a letter documenting your success!
    5. Bring this letter of success and receipts to the 4J Employee Benefits Office for reimbursement.

    Offers eligible students and staff individual cessation counseling.  Ask about classes being offered.
  • Oregon Tobacco Quit Line

    Provides information and referral.  Depending on insurance, refers clients to cessation providers in their area and to the Free and Clear Program cessation program which is accessed by telephone.  There are packets that are geared for youth specifically and there are also youth specialists.

    Smoking cessation support for UO students and staff only.  Offers a cessation quit kit; individual counseling: low cost gum and patches available for students at UO Health Center's Pharmacy.
  • Options Tobacco Cessation Program
    This program is offered free to all 4J employees.  (Anyone can accompany you for the $45 program fee).  Operated by the Oregon Vascular Institute, this program provides:
    • Experienced specialists to assist you with setting goals, enlisting support
    • Individual counseling
    • Support for self-help, including telephone counseling, skills training
    • Behavior therapy sessions to help you understand tobacco dependence, learn stress management, weight management
