4J operates three school-based health centers located at Sheldon, North Eugene and South Eugene High Schools. The centers are staffed with a school nurse, health assistant, nurse practitioner and mental health therapist. The health centers handle most health issues, including illnesses, injuries, well child physicals, immunizations, and ongoing health problems. The centers also provide mental health counseling for depression, anger management, attention and focusing challenges, family stress and change, anxiety, substance abuse.
The Community Centers of Lane County operates the fourth health center at Churchill High School. The health center is staffed by a nurse practitioner, a bilingual/bicultural medical office assistant, a nurse, and a health assistant. The clinic is open year-round and has extended evening hours. Family members are welcome to make appointments for the late afternoon and evening hours. The health center can bill OHP, private insurance and Medicare, and has sliding scale discounts for the adult family members. The number at Churchill is 541-790-5227.
Middle and elementary school students can access services at all of the health centers. In addition to providing health treatment for those unable to afford medical care, the health centers can bill private insurance and OHP. Regional nurses rotate to the elementary and middle schools, and can help students and families access health care at the health centers, set up care plans for students who have health problems, and offer consultation services to the school sites and to families. Health Requirements
- Immunizations (also known as vaccinations, inoculations or shots): In order to comply with state immunization laws, a signed Oregon Certificate of Immunization Status must be completed before a child starts school. Parents must fill out the form, showing compliance with State immunization requirements (including the day, month, and year of each immunization): polio, measles, mumps, rubella, diphtheria, tetanus, pertussis, hepatitis A and B, and varicella (chicken pox) vaccine or date of disease. Tdap vaccination also is required for students in grades seven through ten. If there is a medical reason for the student not to have immunizations, a physician must sign the certificate. The parent may sign a statement opposing immunizations on religious grounds.
- Medical Examinations: A medical exam by a health-care practitioner is recommended for all students entering 4J schools for the first time. If such an exam is a financial hardship, please contact the school nurse or one of the four school-based health centers for an appointment.
- Medications at School: Some health problems may require medication while at school. The school district has a policy that allows school personnel to assist a student with medication only with written permission. Contact the school for information.
- Special Health Problems: Parents or guardians of students with special health problems (students who may require health care during the school day, e.g. diabetes, asthma, seizures, severe allergies) should contact the school nurse. Please do this at least one week prior to the child attending school to allow adequate preparation time.
- Visitors/Home-School Students: Children visiting for more than five days or attending part-time (including home-school students) are required to follow immunization requirements.
