Overview

4J operates three school-based health centers located at Sheldon, North Eugene and South Eugene High Schools. The centers are staffed with a school nurse, health assistant, nurse practitioner and mental health therapist. The health centers handle most health issues, including illnesses, injuries, well child physicals, immunizations, and ongoing health problems. The centers also provide mental health counseling for depression, anger management, attention and focusing challenges, family stress and change, anxiety, substance abuse.

The Community Centers of Lane County operates the fourth health center at Churchill High School. The health center is staffed by a nurse practitioner, a bilingual/bicultural medical office assistant, a nurse, and a health assistant. The clinic is open year-round and has extended evening hours. Family members are welcome to make appointments for the late afternoon and evening hours. The health center can bill OHP, private insurance and Medicare, and has sliding scale discounts for the adult family members. The number at Churchill is 687-3233.

Middle and elementary school students can access services at all of the health centers. In addition to providing health treatment for those unable to afford medical care, the health centers can bill private insurance and OHP. The clinics are open 8 a.m.–4 p.m. during the school year, and can be reached at the following numbers: North Eugene 687-3271; Sheldon 687-3345; South Eugene 687-3209.

Regional nurses rotate to the elementary and middle schools, and can help students and families access health care at the health centers, set up care plans for students who have health problems, and offer consultation services to the school sites and to families. They can be reached via their pagers; please contact the individual school for the number.

Health Requirements

  • Immunizations (also known as vaccinations, inoculations or shots): In order to comply with the Oregon State Immunization Law, a signed Certificate of Immunization Status must be completed before enrolling. Parents must fill out the form, including the day, month, and year of each immunization. This form must show compliance with State immunization requirements: polio, measles, mumps, rubella, diphtheria/tetanus, hepatitis B and varicella vaccine or date of disease (chicken pox). If there is a medical reason for the student not to have immunizations, a physician must sign the certificate. The parent may sign a statement opposing immunizations on religious grounds. Transfer students have 30 days to comply with immunization requirements.
  • Medical Examinations: A medical exam by a physician is required for all students entering 4J schools for the first time. If such an exam is a financial hardship, please contact the school nurse or one of the Health Centers at the four high schools for an appointment.
  • Medications at School: Some health problems may require medication while at school. The school district has a policy that allows school personnel to assist a student with medication only with written permission. Contact the school for information.
  • Visitors/Home School Students: Children visiting for more than five days or attending part-time (including Home School Students) are required to follow immunization requirements.
