Prior Life Insurance and Long-Term Disability Coverage: Administrators

Life/Accidental Death & Dismemberment (AD&D) Information
The District currently provides a basic $50,000 group Life/AD&D Insurance policy for all administrative employees. For more details, please refer to the Basic Life Certificate of Coverage.
In addition, each employee (dependents are not eligible for this coverage) may purchase a $50,000 supplemental life/AD&D policy with a monthly payroll deduction of $9.00. For more details, please refer to the Supplemental Life Certificate of Coverage.
How To Apply
Application During Initial 31 Day Eligibility Period– If you are applying for the $50,000 supplemental life insurance coverage during your initial 31 day eligibility period, indicate this election on your 4J Enrollment Form and return it to the Benefits Office. Coverage is guaranteed during this period so no medical information will be required. If you do not enroll when initially eligible, you are allowed to apply for coverage later, but will be required to submit medical information for underwriting approval and coverage may be denied.
Application After Initial 31 Day Eligibility Period– If you are applying for the $50,000 supplemental life insurance coverage after your initial 31 day eligibility period, complete the Voluntary Benefits Enrollment Form and submit it directly to Regence Life & Health Insurance Company (the address is printed at the top of the form). Regence Life & Health will review the information and send a determination letter directly to you, as well as notifying the 4J Employee Benefits Office. (If coverage is denied, no reason is given to the employer, but details will be provided to the employee.) Approved coverage will take effect the first of the month following the date the Benefits Office receives approval from Regence Life & Health. Associated premiums will not be assessed until approved coverage is in effect. If coverage is denied, this will not affect your eligibility for the Basic Life insurance provided by the District.
Designating a Beneficiary
Designate your beneficiary by submitting the Life Insurance Beneficiary Designation Form to the Employee Benefits Office. This can be changed anytime by submitting a new form.
You may name more than one beneficiary. When more than one beneficiary is named, benefits will be awarded equally among the beneficiaries unless you have designated otherwise.
You may also name a contingent beneficiary, in case your primary beneficiary is no longer alive at the time benefits are paid. See the Life Insurance FAQ's for more information on multiple beneficiaries.
Please note: If death occurs and a minor (a person not of legal age) is the beneficiary, it may be necessary to have a Guardian of the Estate of the minor or a Conservator for the minor appointed before any life proceeds can be paid. This means legal expense for the beneficiary and delay in the payment of the insurance, a point to consider when naming a beneficiary.
How to Cancel Coverage
If you are currently enrolled in the $50,000 supplemental life insurance plan, and wish to cancel coverage, simply make this request in writing to the Employee Benefits Office (email to benefits@4j.lane.edu from your 4J email account will fulfill this requirement). Coverage will terminate the first of the month following receipt of your request. If desired, you may re-apply for this coverage at a later date following the procedures above.
What Is Provided by the Long-Term Disability (LTD) Insurance?
The LTD coverage is designed to provide income protection should you become disabled on or off the job. You may file a claim if you are unable to continue working or if you must reduce your hours due to a disability. How Does the LTD Coverage Work?
If you become disabled and your claim is approved, you will receive up to 60% of your pre-disability gross income up to the monthly maximum of $6,900* beginning 90 days after your disability began. Refer to your Certificate of Coverage for more information.
*Note: The monthly maximum benefit was increased effective October 1, 2008. If you date of disability is prior to October 1, 2008, the monthly maximum benefit in effect at that time would apply to your claim.

Can I get life insurance for my dependents?
No, this benefit is only available to employees.
Administrators Long-Term Disability Insurance FAQ
Is there a limit on the amount of time I am eligible to receive LTD benefits?
Yes. If your disability prevents you from continuing only in your own occupation, the benefits last a maximum of 36 months. If, however, your injury prevents you from working at any occupation, even after the first 36 months, then your benefits will continue per the details of the contract. Refer to the Certificate of Coverage for details.
Does my age at the time of my disability affect the benefits I receive?
Yes. If you are 61 years of age or younger, the maximum benefit lasts until you become 65. After age 61, the benefit lasts varying amounts of time, up to age 69. Refer to the Certificate of Coverage for details.
If I have other sources of income, am I still eligible to receive LTD benefits?
Yes, but there are offsets for other income sources. Refer to the Certificate of Coverage for details.
